		abstract={Public Health Center in Serang district must provide basic services to the society, one of which is the environmental sanitation program in each village. Public Health Center will make a report to the health department, but some report data supplied are not accurate like the number of householder who examined greater than the number of householder that exist, it happens because making the report still manual. The amount of data received from 31 Public Health Center also cause performance of environmental sanitation section of health department Serang district becomes less efficient. The purpose of this study to ease Public Health Center and health department in making the report. In this study the authors use the UCD (User Centered Design) method, the term that use to describe a design philosophy that puts the user as the center of a system development process. In making this application the author using the programming language C# and helped by SQL Server as a container for building database applications. The results of the research that conducted at the health department Serang district, this web application will ease the Public Health Center and Health Departement in making reports anywhere and anytime.}
