According to SDKI, in Indonesia, the Infant Mortality Rate in 2017 was still high with the number of 305/100.000 Live Birth. Meanwhile, it was 7 of 27.876 Live Birth in Palembang in the year 2017. One of the ways to decrease the Maternal Mortality Rate and Infant Mortality Rate is by implementing Midwifery Continuity of Care (COC), continuous care starting from the pregnancy up to the family planning phase. This final project report aims to give Midwifery Continuity of Care (COC) for Mrs. ?L? at Lismarini?s Midwifery Independent Practice beginning on March 07, 2021, encompassing the pregnancy, new-born baby phase, postpartum period, and neonate. The data were gathered from the primary and secondary sources. The implemented method was the approach of Midwifery Continuity of Care and was supported by the SOAP method. The result from giving Midwifery Continuity of Care for Mrs. ?L? showed that the pregnancy, the childbirth from phase I to IV, new-born baby phase, postpartum period, and neonate were still in the normal limit. The conclusion showed that giving Midwifery Continuity of Care allowed the mother?s and baby?s conditions to be monitored, and the emerging problems were able to be detected earlier.} }
