This study aims to determine the relationship between people's perceptions and their motivation to do the COVID-19 vaccine. A cross-sectional design and quantitative correlation were used in the study design. The purposive sampling method was used to collect data from 90 people in Suren Village, Mlarak District, Ponorogo Regency. A questionnaire with a Likert scale is used as an instrument to measure people's perceptions and motivation. By using Chi-Square statistical analysis. The results showed that there was a relationship between public perception and motivation for giving the COVID-19 vaccine in Suren Village, Mlarak District, Ponorogo Regency with a research p-value of (0.004) < (0.05). The findings of this study are expected to change people's perceptions and increase their motivation to receive the COVID-19 vaccine to prevent the spread of the COVID-19 virus chain.
