		abstract={Nursing care documentation is evidence of recording and reporting that nurses have in care records that are useful for the benefit of clients, nurses and the health team in providing health services on the basis of accurate and complete data in writing as the nurse's responsibility. The completeness of nursing care documentation as one of the components in the medical record file is 100%. Negligence of inaccuracies in making documentation will cause the nursing services provided to be unsustainable. The purpose of the study was to analyze the relationship between the level of motivation, training, education, and knowledge of nurses with the completeness of filling out the medical record form of nursing care. This type of research uses literature review research. The results of the research obtained by the researcher that there is a relationship between motivational factors with a percentage of 33.33% of 21 articles, training factors with a percentage of 14.28% of 21 articles, educational factors with a percentage of 19.04% of 21 articles, knowledge factors with a percentage of 28.57 % of 21 articles. The advice obtained is for the health service facilities to provide motivation and evaluation to encourage the spirit and discipline of nurses so that they can improve the completeness of filling out nursing care forms. For the health service facilities to provide education, training, and seminars regarding filling out medical record documents, especially nursing care forms.}
