		abstract={Medical record files at the Nogosari Health Center are easily torn because the filing is full, so it becomes difficult to retrieve medical record files and if forced, the medical record files will become torn and it was found that from 2000 files there were 157 damaged files or 7.35%. This type of research is qualitative. Data collection techniques using interviews, observation, and documentation. The object of this research is the medical record file at the Nogosari Health Center, and the subject in this study there are 5 respondents including 1 registration officer, 1 filing officer, 1 midwife, 1 Head of Administration, 1 Head of Nogosari Health Center. The results[s1]  obtained in this study are based on the CARL (Capability, Accessability, Readiness, Leverage) problem priority method. The first priority problem is found in the material namely it was found that the medical record file was torn at the end of the medical record due to the map material. folio made of paper and measuring 23 cm × 34.5 cm so that an effort was determined, namely the redesign of the medical record file folder. The suggestions for Puskesmas are holding training and seminars to make officers understand more about the importance of managing medical record files, considering that officers do not have medical record backgrounds. Each room is given information about the SOP which is framed or pasted so that every officer can read it and remember it easily.}
