		abstract={One of the indicators for medical records is 100% for Informed Consent completeness. Based on a preliminary study at Jember Lung Hospital, it was found that the average number is at 40,02% of the 102 informed consent files are incomplete. The purpose of this research is to determine the number of incompleteness and the causes of informed consent incompleteness in inpatient surgery. This research uses a qualitative research method carried out at Jember Lung Hospital February to July 2021. The results showed that the high incompleteness of the informed consent was caused about there is no punishment was received by the officer if he did not fill in the informed consent. Doctors and nurses have never received training about medical records. The SOP does not include the number of completeness filling. The knowledge of doctors in completeness standarts of informed consent are less. The conclusion of this research is the incomplete number of informed consent filling caused the Minimum Service Standars of medical record in hospital can not be fulfilled. It is because the performance has not been maximized. Some advice can be applicated is making an SOP of Supervision of Completeness Assessment Accuracy of Informed Consent for enhance about punishment policy, implement training according to medical recording principal to doctors and nurses, doing revision for SOP to add policy of minimum standarts in filling informed consent to determine the number of completeness, and socializing SOP to doctors to enhance information about regulacy that used in Rumah Sakit Paru Jember.}
