Abstract :
Nowadays technology is growing. This makes human Propensity towards higher technology for the benefit of human technology makes it easier o do the activity. Enterprise business processes never out of technology for the development of business solutions that are able to provide more intelligent. Applications Request and Report Data is a software that will help automate the performance of an employee at a company in order to achieve effectiveness and efficiency in the work. Problems found in the user prior to use even this variety of applications, namely a lack of insight into the operation of a program or application, not a system that will automatically help the performance of employees ( manual systems ), less detail process the request and report the data to the company resulting in data loss or destruction of data, the results of the request and report the accumulated data is not automatically Of the existing problems , the writer makes the basis of making the application request and report data that will automatically help the performance of employees in a company , so that the request and report the data will be detailed and well organized so as not to cause a lot of problems as above . As for the design of this application is to use the PHP programming language and MySQL database software notepad + + as supporting. Steps being taken is the planning phase, design phase, coding phase and the testing phase. To test the system that has been designed, the authors use a Black Box testing method
