Abstract :
One of the turbine failures is caused by human negligence that occurs during system operation and ongoing maintenance. Human error is caused by the failure of the electrical power output which can be fatal when operating using the Kongsberg Simulator. Kongsberg simulator is a software application that can simulate a marine steam power plant with an automatic marine system that is used to identify and run a power plant so that it can run well on the sea. To find out the cause of the failure of the steam turbine during operation, it can be searched using the Bow-Tie method. This study uses a qualitative approach because the problems are quite complex and dynamic so that the data obtained are taken from trusted journals and taken from observations using the Kongsberg simulator. In this discussion, the failure parameters that occur in the steam turbine have been determined, namely Overspeed Failure and Governor Failure. The cause of the lack of instability of the Steam Turbine on the electrical power output of the Ship is the imperfect or less stable operation of the Steam Turbine resulting in poor electrical output power. When the tanker operates using the Kongsberg Simulator, it requires a power of 450 kW. The electrical output power requirement is 400 kW, a maximum of 450 kW and normally 444 kW.
