Abstract :
The purpose of this system is to produce a computerized health information system where the previous health service system still using manual system, so it is not efficient and optimal to support the progress and development of health services. By utilizing health care system, can improve performance as well as better data processing, right, fast and accurate. Denkesyah Pematangsiantar Polyclinic has problems in managing registration data, patient data collection, patient medical records, reports to the leadership and drugs delivery to patients, based on existing problems, it is necessary to design a web-based health information service system using PHP and HTML programming languages and MySQL as storage media or database. The system development that writers use is the waterfall methodology. The results of the system development is the system can help Polyclinic of Denkesyah Pematangsiantar to manage registration data, documenting data, patient medical records, drug delivery to patients. This system can assist leaders in getting reports
