Abstract :
Every year more than 300,000 women die from complications of pregnancy, childbirth and 2.5 million babies die during the first month of life. In 2019 around 6,700 daily neonatal deaths. In 2015 the MMR in Indonesia was still 305 per 100,000 live births. One of the Sustainable Development Goals (SDGs) targets MMR is 70 per 1000000 live births and IMR up to 12 per 1000 live births. The midwifery care performed on Mrs. ?T? is midwifery care with continuity of care with a midwifery management approach and documentation in the form of SOAP notes at Ellna Palembang Midwife Independent Practice in 2021.
This final report method uses a case study with primary and secondary data collection techniques by means of interviews, observations and physical examinations and obstetric examinations. The results of COC (continuity of care) midwifery care for Mrs "T" during pregnancy, neonates, and the puerperium were in the physiological scope, but for delivery a cesarean section was performed based on ultrasound results. Suggestions are expected for midwifery service health workers to improve the quality and quality of midwifery services in accordance with service standards that have been applied.
