Abstract :
Data from the World Health Organization (WHO) shows that the
maternal mortality rate (MMR) is estimated at 295,000 people in the world. In
Indonesia, maternal mortality in 2015 was estimated at 305 per 100,000 live
births (KH), the Palembang City Health Office stated that the maternal mortality
rate (MMR) in 2018 was 4 people out of 26,837 live births. Based on the results of
the Indonesian Demographic and Health Survey (IDHS) in 2017, the Infant
Mortality Rate (IMR) was 24 per 1000 live births. As one of the efforts to
overcome Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR),
Continuity Of Care (COC) midwifery care is needed to reduce MMR and IMR.
This final project report provides Continuity Of Care (COC) midwifery
care to Ny. "L" at PMB Ellna Palembang from 07 March 2021 to 10 May 2021
starting from the period of pregnancy, childbirth, new-born baby phase,
Postpartum, and Neonates. The data collection technique is using primary and
secondary data. The case study method used in this final report uses SOAP
progress notes.
The results obtained in Mrs. "L" in pregnancy, childbirth, New-born
baby phase, postpartum and neonates were still within the physiological scope,
family planning studies were not carried out because the patient had not used
contraception.
The conclusion from subjective data and objective data is that the
assessment of normal pregnancy, normal delivery, normal newborn, normal
postpartum, normal neonates. The plan is enforced according to the assessment.
