Abstract :
Based on the data from World Health Organization (WHO), the Maternal Mortality Rate in the world in 2018 were 831 per 100.000 Live Birth, and the Infant Mortality Rate in the world in 2017 were 29 per 1000 Live Birth. Palembang Health Department stated that the Maternal Mortality Rate in 2017 were 7 persons from 27.876 Live Birth, while there were 29 babies consisting of 20 neonate babies (0 to 28 days) and 9 babies (from 29 days to 11 months) from 27.876 Live Birth. The aim of Midwifery Continuity of Care is to decrease the number of Maternal Mortality Rate and Infant Mortality Rate. The Midwifery Continuity of Care for Mrs. ?P? was located at Ms. Lismarini?s place who conducts an independent midwifery practice in Palembang, starting from pregnancy, child-birth, new-born baby, neonate, postpartum period, up to family planning. This study used descriptive method. The data was gathered from the primary data and by observing the secondary data, and was documented by using SOAP (Subjective, Objective, Assessment, Plan) note which was being conducted from January 20 to April 11 2021. The result taken from observing Mrs. ?P? showed that the pregnancy, childbirth, new-born baby, 6 hours and 6 days postpartum period, 6 hours and 6 days neonate, and family planning was at the normal limit and still in the physiological scope. The conclusion suggested that a mother who had normal pregnancy period, although its partograph passed the action line, the child-birth would be normal. There was no problem about the postpartum period, neonate, and family planning, and was still in the physiological scope. Finally, it is suggested to the midwifery field that the quality of the midwifery service should be maintained and kept.
