Abstract :
This Final Project Report was prepared by Maya Sari, student identification number 1941211031, with the title "Income Tax Reporting Procedure Article 21 for Doctors Who Earn Income at Assyifa Sukabumi Islamic Hospital Through E-Form". This report is based on research conducted by the author at the Assyifa Islamic Hospital Sukabumi. Taxes are the backbone for state life which is used for national development. One of the types of taxes in Indonesia is income tax. Income Tax will always be imposed on people or business entities that earn income in Indonesia. Income Tax that applies to employees is Income Tax (PPh) Article 21. In this research method, it uses an observation method where the author obtains data directly and accurately. Where the object is a doctor at the Islamic hospital assyifa Sukabumi. The results of the study explained that the reporting used by the Doctor of the Islamic Hospital Assyifa Sukabumi used the type of form 1770 and reported his tax return through the E-Form application. The delivery is carried out offline by filling in a PDF file that was previously downloaded through Adobe PDF Raider. For now, the Doctor of the Islamic Hospital Assyifa Sukabumi has carried out his obligation, namely reporting income tax article 21 properly and correctly in accordance with the Tax Law.
