Abstract :
Schizophrenia is a psychotic disorder with distortions that cause different mindsets from ordinary people, visual hallucinations are one of the symptoms of schizophrenia that are often found in clients with mental disorders. Hallucinations are often identified with schizophrenia, the client feels seen, heard, smelled, there is a sense of touch and taste even though there is no stimulation aimed at the five senses. The purpose of this scientific paper is to provide nursing care to clients with schizophrenia with nursing problems sensory perception disorders: visual hallucinations which include assessment, data analysis, diagnosis, intervention, implementation, and evaluation of nursing. 03 January to 06 January 2023 in the Abimanyu Room RSJD Dr. Arif Zainudin Surakarta for 4 days with the nursing process approach method.The results of the client's assessment see red shadows, these shadows appear when the patient is alone and when the patient is daydreaming from this condition the client experiences sensory perception disturbances: visual hallucinations. Clients are given interventions with implementation strategies (SP) 1 to 4 for 5 days. From the evaluation results, it was found that the client was able to control hallucinations by rebuking hallucinations, taking medication regularly, conversing and carrying out scheduled activities so that the problem was partially resolved. Clients with hallucinations need good therapeutic communication, in carrying out nursing care the role of the nurse is needed to carry out intensive actions on clients Keyword : visual hallucinations, schizophrenia, nursing care
