Abstract :
Latar belakang: Povidone iodine (PVP ? I) adalah antiseptik yang dikenal dapat
membunuh atau menghambat pertumbuhan mikroorganisme, banyak tersedia
diberbagai negara dan relatif murah. PVP-I 5% dinyatakan aman dalam beberapa
penelitian akan tetapi efikasi sebagai obat tetes telinga dalam tatalaksana Otitis
Media Supuratif Kronik (OMSK) belum ada kejelasan. Adanya masalah
perubahan pola kepekaan kuman terhadap antibiotik dan biaya pengobatan cukup
mahal, dirasa perlu dilakukan penelitian untuk membandingkan efikasi tetes
telinga PVP ? I 5% dengan tetes telinga antibiotik dalam tatalaksana OMSK.
Metode: Penelitian ini menggunakan telaah sistematis dan meta analisis.
Pencarian literatur Randomized Controlled Trial efikasi tetes telinga PVP ? I 5%
dibandingkan dengan tetes telinga antibiotik dalam tatalaksana OMSK dari
database PubMed, Springerlink, Cochrane dan Google Scholar. Data yang
dikumpulkan adalah angka kesembuhan. Hasil: Teridentifikasi 407 artikel
penelitian, setelah dilakukan penyaringan tersisa 24 literatur. Akhir penyaringan
tersisa 2 literatur kemudian dilakukan review menghasilkan 2 literatur untuk
dianalisis. Analisis gabungan nilai risiko relatif (RR) kesembuhan kedua
penelitian adalah 1,06 (95% CI: 0,86 - 1,30), nilai Z=0,54 dan p=0,59 (>0,05)
sehingga baik secara klinis ataupun secara statistik tidak ada perbedaan bermakna
kesembuhan penderita OMSK setelah diberikan tetes telinga Povidone Iodine 5%
dibandingkan dengan tetes telinga antibiotik. Simpulan: Penggunaan tetes telinga
Povidone Iodine 5% dan tetes telinga antibiotik memiliki efikasi yang sama
baiknya dalam tatalaksana OMSK. Perlu evidence lebih banyak tentang tetes
telinga PVP-I 5% dalam tatalaksana OMSK.
Kata Kunci: Otitis media supuratif kronis, povidone iodine, tetes telinga, terapi. / Background: Povidone iodine is an antiseptic known to kill or inhibit the growth
of microorganisms, widely available in various countries and relatively cheap. 5%
Povidone iodine is safe in several studies, but the efficacy as ear drops in the
management of CSOM is not clear. There is a problem of changing the pattern of
sensitivity of germs to antibiotics and the cost of treatment is quite expensive, it is
necessary to conduct a study to compare the efficacy of 5% povidone iodine ear
drops with antibiotic ear drops in the treatment of CSOM. Methods: This study
used a systematic review and meta-analysis. A literature search for Randomized
Controlled Trial the efficacy of 5% povidone iodine ear drops compared with
antibiotic ear drops in the management of CSOM from the PubMed, Springerlink,
Cochrane and Google Scholar databases. The data collected is the cure rate.
Results: Identification of 407 research articles, after screening the remaining 24
literatures. At the end of screening, the remaining 2 literatures were then
conducted a review to inlcude 2 literature for analysis. The analysis of the relative
risk value (RR) of recovery in the two studies was 1.06 (95% CI: 0.86-1.30), the Z
value = 0.54 and p = 0.59 (> 0.05). Therefore, both clinically and statistically
there was no significant difference in the recovery of CSOM sufferers after being
given 5% Povidone Iodine ear drops compared to antibiotic ear drops.
Conclusion: The use of 5% povidone iodine ear drops and antibiotic ear drops
have the same efficacy in the management of CSOM. More evidence is needed
regarding PVP-I 5% ear drops in the management of CSOM.
