Abstract :
Based on data from the World Health Organization (WHO), maternal mortality is death during pregnancy or within 42 days. Maternal Mortality Rate (MMR) is the ratio of maternal mortality during pregnancy, childbirth caused by pregnancy, delivery, and Postpartum or its management but not due to other causes such as accidents or falls in every 100,000 live births. Based on Indonesia's health profile, there has been a decrease in maternal mortality from 1991-2015, from 390 to 305 per 100,000 live births. The Maternal Mortality Rate in NTB Province during 2017 was 85 cases. It was a decrease compared to 2016 with 92 issues.Health Profile of the West Nusa Tenggara Province noted that the number of maternal deaths in the West Nusa Tenggara Province during the last five years had decreased. During the period of 2013-2017, there was a decrease in maternal deaths in the West Nusa Tenggara Province about 32 people. In the same period, the average reduction in the number of deaths reached 8.45% per year. In Indonesia, 75% of women have Perennial lacerations who give birth vaginally. In 2017 it was found that out of a total of 1951 spontaneous were vaginal deliveries, 57% of mothers received perinea sutures (28% for episiotomy and 29% for spontaneous tearing. The purpose of this case study is to make students be able to provide Midwifery Care to Postpartum mothers with Grade II Perennial Tearing at UPT BLUD Gunungsari Health center.The method used is a case study that was carried out since June 25, 2020, with Grade II perennial Tearing. The data collection techniques used were interviews, observation, and physical examination. Based on the results of this case study, it shows that the same treatment is given to the patient, Mrs. "S" by providing KIE regarding prevention and analysis. In this thesis, there is no gap between theory and practice on subjective, objective, analysis, and management data. It is hoped that mothers and their families can make efforts to prevent the worst possibility caused by grade II perennial tearing by maintaining a healthy lifestyle, personal hygiene, and routine checkups.
