Abstract :
PT. XYZ is a subsidiary of a State-Owned Enterprise (BUMN) that engage in port services. One of the business focuses of the company is in the field of logistics business, PT. XYZ itself has indicators in measuring the productivity of company services, namely the quality target of the loading and unloading process (receiving, stripping / stuffing, and delivery). In the loading and unloading service itself, PT. XYZ has the disadvantage, there is a decrease in the loading and unloading service time, the decline has a difference in impact with the company's service quality standards. The purpose of the research is to identify the cause of the decline in service productivity, as well as to provide suggestions for the improvements or recommendations to the company in overcoming the problem using Fault Tree Analysis (FTA) and Failure Mode Effect & Analysis (FMEA) methods, in providing the proposal or recommendation the researcher can help PT. XYZ in improving the loading and unloading services by looking at the basic FTA event, then used as points in the form of FMEA, and in calculating its RPN by sorting points with the highest RPN value. This research is expected to provide solutions to problems that occur in the company.
