Design of Assessment Form and Informed Consent for Mental Disorders Patients to Support the Mental Health Services Unit of Candipuro Health Center, Lumajang Regency, Ferly, NIM G41161176, Year 2020, pg, D-IV Medical Record, Health, State Polytechnic of Jember, Ida Nurmawati, S.KM., M.Kes (Supervisor I) Ferly Medical Record Study Program Department of Health ABSTRACT The medical record form is an important means of communication because it contains detailed data about all actions performed by medical personnel. Based on a preliminary survey at the Candipuro Public Health Center, Lumajang Regency, the assessment form and informed consent of mental disorders patients is only a sheet of paper, the contents of the form do not include information that can support the enforcement of a psychiatric diagnosis. The purpose of this study was to design an assessment form and informed consent of patients with mental disorders at the Candipuro Health Center Mental Health Service Unit. The method used was qualitative with data collection techniques of observation, interviews, and brainstorming, the research subjects were 1 medical record officer, 2 mental patient nurses, a doctor who handled mental disorders patients, and the head of the puskesmas. Data analysis in this study used data reduction analysis, display data, and conclusions. The results of this study are the design of the assessment form design and informed consent of mental patients according to user needs based on the physical aspects using HVS F4 paper weighing 70 grams, the anatomical aspects adjust the form design standards, and the aspects of the form contents adjust the user's needs by including the results of the history of the patient with the disorder. mental (alloanamnesa and heteroanamnesa), psychosocial history, mental status history, physical examination, medical diagnosis, therapy, and follow-up services. The design of the assessment form and informed consent of patients with mental disorders can be applied in the Mental Health Service Unit of the Candipuro Community Health Center to support the completeness of information on the medical records of patients with mental disorders.
