Abstract :
Based on the result of preliminary study at RSUD Genteng, it was found that there was a build up of medical record documents in the inactive room that caused by non-execution of destruction which resulted in damage inactive medical record documents and difficult to find. If the destruction of medical record files is carried out, inputting a list of file descriptions using Microsoft Excel then scanning of files that cannot be destroyed is stored in a folder on the computer. The impact in the application of using Microsoft Excel is lack of the security system, hard to control, manual data processing and reporting so that it is difficult for the staffs to provide effective and efficient services to the patients. This study aims is to design and build a web-based information system for depreciation and destruction of medical record documents at RSUD Genteng. The method used is prototype method which is the stages of identifying user needs, build prototype, evaluate prototype, coding prototype, system test, system evaluation, and use the system. PHP web programming language and MYSQL database combined with Bootstrap templates. The result of this study is that the existence of this information system can make it easier for the staffs to carry out the destruction of medical record documents and can improve data storage.
