Abstract :
Medical record borrowing and returning information system is a computerized process for recording transaction between borrowing and returning medical record. Medical record are confidential documents, so it has been stored well for controlling the transaction of medical record. Kedopok community health center already using Microsoft Excel, so the record stiil not integrated and it takes a long time to write it. This process make ineffectiveness and inefficiency to make a reports of borrowing an returning medical record. Therefore, it?s needed information system to solve the problem that occur in Kedopok Community Health Center with barcode technology dan late returning warning alert. This research is qualitative with data collection method are interview, observation, documentation, and brainstorming. This use method waterfall development, where this method have any process are analysis requirement software, design, coding, and testing. Analysis requirement software is process of analysis the problems and requirement of designing the information system. Design system using flowchart diagram, context diagram, Data Flow Diagaram, and Entity Relationship Diagram. Coding step using HTML, PHP, bootstrap, MySQL, and codeigniter. And the last step is testing that used black box testing for checking the function in borrowing and returning information system. The result of research is an information system of borrowing and returning medical record that can help user to make easy and quickly user? works.
