Abstract :
Filling in the code in the inpatient medical record file of the Kalisat Regional Hospital, Jember Regency is incomplete. In the first quarter of 2019, the files that experienced 20% incompleteness were contrary to the Ministry of Health No. 129 of 2008 which stated that the patient's medical record files must be 100% complete. Incomplete filling of medical record files can affect further activities, namely coding activities. This study aims to analyze the factors causing the incompleteness of inpatient medical record files using Lawrence Green's behavior theory. This type of research uses qualitative. Data collection techniques by interview, observation, documentation and questionnaires. The results of the research obtained by the researcher are not good knowledge and attitudes, the work environment is less comfortable, there is already an SOP regarding the completeness of the file. Efforts made are attending seminars or training, improving work space layout or adding facilities, and disseminating SOPs related to completeness of inpatient medical record files.
